All Texans residing in covered counties that meet eligibility requirements and complete a video registration or attend a seminar and complete a Consent-to-Contact form may receive a complimentary gift card with no obligation to enroll. Limit one (1) gift card per eligible completed form between October 1, 2023 and December 1, 2023. Offer valid while supplies last and void outside of covered counties, and where otherwise prohibited. Gift card cannot be redeemed for cash.
